Personalized Care

Plan out your meals for the week

Having your meals and snacks planned out for few days to a week saves time and keeps accurate measures of blood sugar levels. How many times a week do you cook at home?

Keep a food diary

Write about what foods help maintain your energy. What is your go-to food when you need your 15 grams of carbs?

Track you blood sugar levels

According to the CDC, medication and physical activity affects blood sugar levels. How are you maintaining your blood sugar level with medication, diet and physical activity?

Daily Physical Activity/Exercise

The American Diabetes Association stated that at least 150 minutes a week is the target for exercise.  You may choose from many activities such as swimming, jogging, walking, and yoga. Make a list to complete different  physical activity/ exercises. Always carry at least 50 grams of Carbs along with you (15 grapes or a sports drink).

Join a support group

 Find a community that can give emotional support or continue to share current information. Support groups can keep you informed about new research and possible studies.

See the Doctor frequently

Keep your Doctor well informed. Let the Doctor know of any changes and issues that affect your total health.
